Assessment Question Types and Examples

This article describes each of the Assessment Question type options available in the LMS Assessment Tool.

Note: A new feature for ALL question types is the ability for Administrators to include a video, image, PDF or other file type on any question. Students will be able to access the uploaded file when answering the assessment question.

Admins can upload the image by selecting  the Attach Document, Image or Video link when creating a new question:

Multiple Choice

Admins enter a single question and multiple answer options. Numerous answer choices can be set as correct, so users can select all that apply.

Multiple Choice (single answer)

Admins enter a single question and multiple answer options. Only one answer choice can be selected as the correct one.

True/False

Admins enter a single statement and users must decide if the statement is True or False.

Sequence

Admins enter a single question and chronological answer choices. Users must then place the answer choices in the correct sequential order.

Note: Admins must place answer choices in correct sequential order when creating a new Sequence Question. When users are taking the assessment, the order of the answer choices will be randomized and users will have to drag and drop to place them in correct order.

Matching

Admins enter a single question and pairs of corresponding answer choices. Users must match the second column of answers to their corresponding matches in the first column.

Note: As with Sequence Question types, Admins must enter answer choices with correct matches. Choices will then be randomized when users take the assessment.

Grid

Admins set up answer choices built around a grid template. The 'Row' labels are the items you wish to be rated. The 'Column' labels are the ratings the user will choose from.

Note: Grid questions do not count towards the Assessment score.

Grid (column values)

This question type is the same as the Grid type except numerical values are assigned to each column (from 1 to the total number of columns). The numerical values are an additional metric to use when running an analysis report on users' responses.

Note: Grid (column values) questions do not count towards the Assessment score.

Rank

Admins set up a single question or statement. They can then add 'Rank options,' allowing users to rank the question or statement on a numerical scale (From __ to __,) as well as corresponding Labels (From least to most, from most agree to least agree, etc.).

Note: Rank questions do not count towards the Assessment score.

Text (short)

Admins provide a statement or question and users are allotted a short text box in which to answer. Suggested answer length for this question type: single word or sentence.

Note: Text (short) questions do not count towards the Assessment score.

Text (long)

Admins provide a statement or question and users are allotted a long text box in which to answer. Suggested answer length for this question type: Multiple sentences or paragraphs.

Note: Text (long) questions do not count towards the Assessment score.

Video

Admins provide a statement, question, or scenario in the question field. In response, students record a video using their device's webcam.

Note: the video software built into the LMS will require the student to 'allow' access to their device's webcam before an answer can be successfully recorded. The video software will ask this automatically in the video field, and the student will need to click 'Allow.' Additionally, after recording their response, the student will have to click 'Accept the Recording' on their video window before submitting. They can also choose to re-record their response. Both options automatically pop up after the recording has finished buffering.

Note: Video questions do not count towards the Assessment score.

Upload

Admins provide a single question, answer, or scenario in the text box. The student answers by uploading any kind of document (Word, image, PDF, etc.)

Note: Upload questions do not count towards the Assessment score.
